Criteria for Referring Clients to CPM 
Patients should be referred for an assessment when any of the following criteria are met.
Immediately after a car accident, work place injury or event when: 

• There is evidence of anxiety while driving, acute stress reactions or PTSD to an event or mechanical low back pain
• Assessing the patient for the risk of developing chronic pain if there is a past history of victimization, other chronic pain behavior or history of chronic pain from other source
• There is suggestion of possible traumatic brain injury at time of impact.
Six weeks after the car accident, work place injury or other accident or event when:
• The patient continues to experience significant pain
• There are indicators of other psychological factors including depression, anxiety or family issues
• The patient is not getting better
• Work or activities of daily living are compromised
Twelve weeks or more after the car accident, work place injury or other accident or event when:
• The client is having difficulty returning to work or increasing activities of daily living
